| What is the parking like at Cooper Park? Parking at Cooper Park is extremely limited. We STRONGLY advise carpooling as much as possible...fill those mini vans! Parking will be available at Vineyard Church on Saturday ONLY. Please be respectful and DO NOT PARK AT THE CHURCH ON SUNDAY! |

| The application form does not look right on my screen (overlapping drop down boxes, long rows over the input fields, etc.) What is going on? First, you should let the ENTIRE form load BEFORE you scroll. It is a big form. In short, your computer does not have enough memory (RAM) available to render the form correctly. Here is the solution that will work in most cases: 1. Clear the cache (temporary files) in Internet Explorer (Usually Tools>Internet Options>Remove Temporary files 2. Shut down Internet Explorer 3. Restart the computer 4. Open JUST MS Internet Explorer. 5. Go to Web site, fill out form. In most cases, this will solve the problem.... |

| Where is registration and what is required at registration? ***REGISTRATION DETAILS-IMPORTANT*** The directions for registration are listed below. We will offer three types of registration. You can scan and e-mail in your documents, mail in your documents or register on-site. On-Site registration will occur Friday, November 4 from 6-9 p.m. at the Embassy Suites 2700 Corporate Exchange, Columbus, OH 43231.. All of the options require the following documentation: Two Copies of your official, stamped and signed team roster Approval to travel form for all teams outside of Ohio South All player, coach and guest player cards Approved guest player form Medial Consent Forms (your own or those listed on your Ohio South player form) COSA Fall Classic Liability Form signed by all players and guest players Mail-In Registrations are to be mailed to the following address: Attn. Registrar, 921 Eastwind Dr., Suite 114, Westerville, Ohio 43081. Mail in registrations must be received by Saturday, October 22nd. The same items listed above must be copied and mailed. The player, coach and guest player cards must have both the front and the back of the cards copied as part of the documents that are mailed. Do not send your actual player, coach and guest player cards, just the front and back copies of the cards. E-mail Registrations are to be e-mailed tocosateamohio@yahoo.com. The same documents listed above must be e-mailed no later than October 22nd. Again, the front and back of the player, coach and guest player cards must be scanned and e-mailed along with scanned copies of the roster, travel form (if needed), guest player form (if needed), liability release form, medical consent forms. |
